Clinical governance is a systematic approach to maintaining and improving the quality of patient care. In quality improvement projects such as a complete loop audit, interventions are often evaluated retrospectively. This presents several challenges including background changes known as secular trends. Techniques to address this issue include cluster randomised controlled trials, interrupted time-series analysis and segmented linear regression methods. These have their strengths and weaknesses and it is important that orthopaedic surgeons understand their underlying principles. Read more...

Size of greater tuberosity fragment: a risk of iatrogenic injury during shoulder dislocation reduction PURPOSE: Shoulder dislocation with greater tuberosity fractures (GTF) is becoming increasingly common, as is the number of cases of iatrogenic humeral neck fractures (IHNF) during reduction. Article of the Month: SICOT-J Periprosthetic joint infections after total hip replacement: an algorithmic approach An algorithm for managing periprosthetic joint infections (PJIs) after total hip replacement (THR) surgery using a multidisciplinary approach and a clearly defined protocol may improve infection eradication rates. In this article, we present an algorithm for the management of different types of PJIs including the acutely infected cemented and cementless THRs where the components are well-fixed postoperatively and when the infection is secondary to haematogenous spread in previously well-functioning and well-fixed implants. For chronic PJIs where the components are often loose, the standard treatment includes a two-stage revision procedure. However, in a highly selected subset of patients, a single-stage approach has been utilised with high rates of eradicating infections. Read more... Case of the Month A 25-year-old male patient involved in a road traffic accident sustained multiple injuries as follows: Closed-head injury, concussion of the brain, chest contusion, bilateral displaced fractures of the pubic and ischial bones alongside injury to the left and right sacroiliac joints, open comminuted fracture of the right femur, open comminuted fractures of the right 4th and 5th metacarpals and multiple lacerations. For many years, the SICOT organisation has had a very successful impact in Egypt. A continuous cooperation has been established with many Egyptian bodies that has led to mutual benefits. One example is the cooperation in the field of fellowships. SICOT approved the training of African SICOT members in two of the largest university hospitals in Egypt; Alexandria University in the north and Assiut University in the south. SICOT also granted a special fellowship programme abroad for members of SICOT and the Egyptian Orthopaedic Association (EOA). Accordingly, many young Egyptian surgeons have received an exceptional overseas training opportunity in centres of their choice. Read more...
